Деякі питання антибактеріальної терапії в ревматології

Важливою проблемою сьогодення є боротьба з інфекційними агентами. Лікування інфекційної патології ревматологічних хворих часто є складним завданням. Основні проблеми, з якими стикаються лікарі‑ревматологи, — це наявність нових збудників захворювань, роль умовно‑патогенної мікрофлори та збільшення впливу резистентних мікроорганізмів, питання ефективності та взаємодії антибактеріальних засобів з протиревматичними препаратами. Усе це зумовлює необхідність пошуку і розробки нових схем та методів антибактеріальної терапії в ревматології. Основними захворюваннями, які асоціюються з бактеріальною інфекцією, є реактивні артрити, Лайм‑артрит на тлі бореліозу Лайма, бактеріальний (септичний) артрит тощо. Проаналізовано джерела літератури та міжнародні рекомендації щодо можливості застосування антибактеріальних препаратів при реактивному артриті, Лайм‑артриті та бактеріальному (септичному) артриті. Наведено схеми раціональної антибактеріальної терапії. При реактивному артриті важливе значення має своєчасне лікування хламідійної урогенітальної інфекції. Відзначено ефективність лікування ранніх стадій Лайм‑артриту, тобто на етапі мігруючої еритеми, що може бути первинною профілактикою ураження суглобів. Висвітлено комбінації та терміни проведення антибактеріальної терапії при бактеріальному (септичному) артриті. Слід пам’ятати, що бактеріальна інфекція може бути тригером розвитку ревматологічної патології, тому своєчасне застосування адекватної антибактеріальної терапії може бути первинною профілактикою окремих захворювань. Тому розробка ефективних схем раціональної антибактеріальної терапії та моніторинг інфекційних агентів є актуальними проблемами. Лікарі‑ревматологи мають бути ознайомлені з новими рекомендаціями щодо менеджменту інфекційної патології та своєчасно призначати максимально ефективні схеми і програми лікування.
